Patient Access to Laboratory Results
Effective October 6, 2014, the HIPAA regulations changed to allow patients to directly request laboratory results from the laboratory that performed the testing. Galaxy Diagnostics, Inc. has developed a Patient Results Request Form for you to complete and submit. The purpose of the form is to ensure that our laboratory results are released to the patient or legal representative of the patient. In this way, your personal health information is protected. We aim to provide the final laboratory report within seven business days of receipt of the completed form. Please note that medical laboratories are not required to provide clinical interpretation of the results to patients. You are strongly encouraged to discuss your laboratory results with your health care provider.
